The brain damage that accompanies aneurysmal subarachnoid hemorrhage (aSAH) is a major cause of mortality and morbidity. Although a large number of clinical and basic researchers are currently studying the mechanisms involved in vasospasm and the disability of these patients, the causes of this
medical problem are not fully known.
Recently, several studies have attributed to the inflammatory response of the arterial wall a role in the SAH, causing changes in the regulatory mechanisms of vascular tone. Multitude of factors are likely to be involved in the regulation of the vascular tone and the inflammatory response. A wide variety of biological processes can trigger vasospasm, which is the main complication of aSAH. One of
them is the inflammatory process that is associated to SAH, constituting itself a possible therapeutic target in the treatment of vasospasm. Inflammation is a complex and multifaceted response that
originates ultimately against foreign antigens. Leukocytes are important components of the inflammatory response, and may affect vascular smooth muscle directly or through the development and propagation of
the inflammatory response. The recruitment of leukocytes to the sites of inflammation is caused by multiple signaling cascades between leukocytes in the vascular lumen and the endothelial cells of the
vessel walls. It has been observed that cytokines might function to coordinate innate immune responses and to affect adaptive immunity, although the mechanisms underlying these effects have not been defined. Neopterin is a central cytokine involved in immune cell activation. It is produced by
macrophages in responses to gamma-interferon. Activated T cells can also synthesize and secrete high levels of Neopterin into body fluids of patients suffering inflammatory or infectious processes. It is
known that Neopterin stimulate the proinflammatory activity by eliciting the chemotactic migration of leukocytes and their adhesion to endothelial cells. Therefore, monitoring serum Neopterin could constitute an early biomarker of infectious processes that occur after SAH.
Chemokines are cytokines that stimulate proinflammatory activity by eliciting the chemotactic migration of leukocytes and their adhesion to endothelial cells. MPIF-1 has chemotactic activity for
monocytes/macrophages, dendritic cells, lymphocytes, and endothelial cells. MPIF-1 is expressed in activated monocytes, and its levels are increased in fluids from patients with inflammatory processes.
MPIF-1 is a biomarker for some inflammatory diseases, including chronic rhinosinusitis, atherosclerosis, systemic sclerosis and rheumatoid arthritis, among others. In addition, it has been reported that MPIF-1 can induce endothelial cell migration, tube formation and angiogenesis through chemokine receptors (CCR1). However, MPIF-1 levels have not been studied in patients with SAH. As vasospasm has an important inflammatory component, it may constitute a plasma biomarker in patients with SAH.
Therefore, the measurement of serum neopterin and MPIF-1 will be determined as biomarkers of the infectious level and the degree of vasospasm in patients after SAH. A comparative study between these biomarkers and the therapeutic approach applied to these patients would help to optimize the treatment of SAH.
